Her father and grandfather happened. They spoon-fed her their twisted, sick, and depraved ideology from the cradle. She was brainwashed, and from what we can tell, she never recovered. (if you've read part 1 of 'The Search' don't tell me anything, because I haven't read it yet)

I say its a matter of genes, a slight mutation that started with Sozin that ended up manifesting in ever more extream ways in the next generation. The worst of it came with Ozai and Azula, Zuko was also stricken with this gene but unlike Azula who was pretty much raised by Ozai and encouraged in her Sociopathic tendencies, Zuko was first cared for by his Mother Ursa, and then had his Uncle who genuinely cared for him, so the gene was off-set by his environment.

I also suspect that since Magic pretty much exists in the Avatar world, then Doing Evil deeds or Having desires that are Evil manifest themselves in the people who commit them, sort of like Demon possesion or something.
